Continuity Care
Continuity Care is located in Winnipeg, Manitoba. It was founded in 1996 by a group of families and concerned citizens. It offers several excellent family-oriented programs including transition planning and sibling groups. Its expertise is especially in helping people build personal networks, which they call Sharing Circles.
